4.1 Evidence status for the Anti-Xa chapter
This chapter is laboratory-standard-supported and clinically source-aware. Numeric treatment targets and reversal decisions must be checked against the local laboratory method and current institutional anticoagulation policy before patient-level use.
The central interpretive message is supported by current guidance: direct oral Xa inhibitors may interfere with heparin-calibrated anti-Xa monitoring after recent exposure. ISTH SSC communication on direct Xa-inhibitor interference and the British Society for Haematology guideline on heparin and non-coumarin anticoagulants provide useful starting points for local policy review.
4.2 Editorial rules for future chapters
Every new chapter should include:
